Introduction

These forms are for GMs and COs to submit information to the GMD regarding when missions start, end, or otherwise change. This information is very important for the GMD and the Mission Archive, as it is through this information that GMs are classified to determine who is eligible to do what. It also serves as a guide to COs looking for GMs to see how prospective GMs have performed in the past.

The forms below are intended to be submitted by STF GMs, but relevant information may also be offered by COs when appropriate. All information is automatically mailed to the STF Librarian for archival.

Mission Comments

GMs, COs, XOs, and FComms may optionally submit "debriefing" info regarding a mission. Such information could include comments on the performance of the GM, mitigating circumstances that the GMD should consider when considering this mission, and so on. These comments may be postive or negative, as appropriate. Please do not submit a comment unless you are a related GM, GMM, CO, XO, or FComm.

Change of GM

If a GM changes mid-mission, including when a GMT is removed and the GMM takes over, the GMD should be notified of the change. It is very important that the GMD have a record of who GMs where, when, and for how long.
