Sickbay - The Headache

For the last three days V’Lath had been afflicted by a nagging headache. He was loath to take any kind of pain medication and the pain seemed to be right behind his eyes. At first he thought that perhaps the air filtration system in his quarters was malfunctioning, but that was checked and operating efficiently. He finally decided he would see a doctor as the pain was not easing at all and in fact was getting worse. So he now found himself in the outer environs of the sickbay, looking for assistance.

OOC: Leonora and Christopher, come in on this with me so we have a place to play.

Sage was walking through medical when she saw V’Lath enter. “Hello, Counselor. Something I can help you with?” She noted the stress lines around his eyes and the set of his jaw. It was a look she was familiar with—pain. “You don’t look well, why don’t you come have a seat over here.” As she led him to a biobed, she caught sight of T’Lora and gave her a come here with a head gesture.

V’Lath followed the doctor over to the biobed and sat down, ” I have had this pain the last few days, right behind my eyes then down the back of my neck. If I didn’t know that Vulcans don’t get them I would call it a migraine, at least I think that’s the term.” V’Lath massaged his temples and eyes. “It will fade for a while, then come back even worse. Meditation techniques seem to have no effect.” He tilted his head back and rolled it side to side, promptly passing out and collapsing on the biobed.

T’Lora had already been making her way over, but when V’Lath fainted, she all but ran the rest of the way. “Dr. Brennan?” She looked to the more experienced Doctor for guidance, unsure if V’Lath had said something about his condition before she could hear.

As Sage fired up the scanner on the bed, she checked V’Lath’s breathing. His airway was unobstructed, so the women settled him more appropriately on the bed and she filled T’Lora in. “He was complaining of pain at the back of his eyes that traveled down the anterior side of his neck.” She watched the monitor for any clues to what had happened. “He actually likened it to a migraine.”

OOC: Aneurysm in the frontal cortex with leakage and pressure causing the pain behind the eyes, with nerve damage causing the pain running down the neck ???

T’Lora frowned, “Vulcans do not have migraines.” Her mind was racing as she ran through conditions with similar effects. “TBI’s can cause migraine-like symptoms. Swelling in the brain, obstruction of blood vessels…” She felt something akin to a chill run down her spine. Brain injuries could be particularly debilitating to Vulcans.

Sage ordered a detailed scan of V’Lath’s head and neck. As the image materialized, she hmphed. “Look at that. An aneurysm in the frontal lobe of his brain.”

—Sage

“Has it burst?” T’Lora studied V’Lath disapprovingly. He had clearly been ignoring the symptoms.

Sage enhanced the image and enlarged it. “Nooo…but it appears to be leaking blood into the brain tissue.” She looked at the Vulcan doctor across from her. “It’s causing pressure to build which is probably why his headache hasn’t gone away, but only gotten worse. We need to deal with it before there is irreparable damage.”

OOC: Are we clipping, removing and sealing, or something else?

OOC: You can go ahead and take the lead on this one. We can do whatever you like. I am dealing with some RL issues at the moement.

OOC: Alright. Hope everything clears up for you. Flow diverters are fairly recent (I didn’t learn about them till after med school,) but I figure by Star Trek time they’ve figured out how to place them fairly easily.

T’Lora nodded decisively. “I’ll prepare to set a flow diverter.” She moved to the cabinets to prep a diverter and microcatheter.

Sage nodded and continued monitoring V’Lath’s vitals. If they placed the diverter properly, they wouldn’t have to enter the aneurysm at all, which was the most dangerous part of any surgery inside the brain.

V’Lath was dreaming, and what was strange was that he knew he was dreaming. He was back on Vulcan and standing outside a dwelling. He appeared to be younger and he carried a satchel of books and scrolls. He proceeded up the walkway toward the building when the door opened and a woman appeared, she was tall and slender with dark hair and as she noticed him she smiled, “More books and scrolls, all you do is study and meditate. You should take a day or two to relax and enjoy yourself.” She came up to him and took his hand, leading him toward the door. She was familiar, he knew her, “Pri’sai is it you ?” She tilted her head to one side, “Now I know you have been studying too hard, who else would it be ?” As they neared the door it opened and a small child ran out, a young girl who ran up and grabbed his other hand, “Father, do you have time to help me with my school work ?” This was surely a dream and he knew it but emotions he could not suppress overcame him.

On the bio-bed the form of the tall Vulcan lay perfectly still as a tear ran down his cheek and softly dropped onto the coverlet, soon followed by another.

OOC: Well, I didn’t think you could dream while unconscious, but apparently it is possible. Who knew?

IC:
Sage made a few changes to the ongoing brain scans to measure electrical output. She’d noticed the moisture from V’Lath’s eye and thought the Vulcan might be in pain. Instead, the readout hinted that he might be dreaming. “I’ve never seen this before.”

Taking the back of her fingers, she wiped away the tears, gently. “The scans say there’s a good possibility our counselor is experiencing some kind of dream, and what ever it is, it is either very sad or very joyful. His right eye is tearing.”

T’Lora blinked confusedly, setting down the tray and hurrying to her side. “We don’t… dream. A memory perhaps, brought forward by the increasing pressure?” She let out a frustrated sigh. “I’ve never seen this before.” She shook her head as if to dislodge the thought. “We will be able to enter the arm with the microcatheter, and presumably be able to place the diverter easily.” She had chosen a pipeline diverter, the most tightly woven, to hopefully prevent it from having to be replaced.

Sage began to cut V’lath’s clothing from his upper body until his chest and arms were bare. =/\=Computer, engage sterilization field over biobed 3. A slight humming let them know the field had been erected over the body of the counselor. Anything that passed through the barrier would automatically become sterile before it touched the patient’s body. “There will be time to discuss the dream or memory later. Are you ready to do this?” She looked at T’Lora encouragingly.

OOC: No problem, I was crazy busy all week, I probably would have lost it too!
IC:
T’Lora nodded, determined. She carefully attached the diverter to the end of the catheter, nodding for Sage to open his arm. “I’‘m ready.”

With a practiced hand, Sage made an incision in the counselor’s upper arm with a laser, cauterizing as she went and using forceps to hold the the slit open. T’Lora would need to feed the micro catheter through the brachial artery to place the diverter in V’lath’s brain.

OOC: Anything going to happen, or is this a smooth surgery?

T’Lora began to insert the microcatheter, keeping a careful eye on the scans as she proceeded through the artery.
